Initially - I'd say the system.  Most systems come with an inherent
setting and flavour.  The exceptions being some of the free generic
rulesets.

However - once the GM is familiar with the rules (and/or the players
are) and they wish to experiment.  Or - if the GM has a set goal for
a campaign.  THEN other subgenre types are likely to be used.
It's usually at this stage that problems with some systems are found
(and no, I am not going to bother bashing D&D again)
